Patient Qualifications
- Adults - ages 18 through 64
- Baldwin County resident
- No private medical insurance, VA benefits, Medicaid, or Medicare Parts A or B
- Proof you are unable to afford Health Insurance
- Proof of income
A diagnosis with high blood pressure, diabetes, elevated cholesterol, pulmonary disease, thyroid disorder, high cholesterol, GERD, or asthma is a requirement to apply.
To Become a Patient:
- Call the clinic office at (251) 937-8096 for screening
- Complete a phone screening
- Provide requested documents
Patients are seen by appointment only; no walk-ins are accepted.
